  Like other calcium channel blockers, amlodipine acts by relaxing the smooth muscle in the arterial wall, decreasing total peripheral resistance and hence reducing blood pressure; in angina it increases blood flow to the heart muscle. Blood pressure is the force of blood pushing against the walls of arteries as it flows through them. Arteries are the blood vessels that carry oxygenated blood from the heart to the body´s tissues. As blood flows through arteries it pushes against the inside of the artery walls. The more pressure the blood exerts on the artery walls, the higher the blood pressure will be. The other name of hypertension is high blood pressure.

The size of small arteries also affects the blood pressure. When the muscular walls of arteries are relaxed, or dilated, the pressure of the blood flowing through them is lower than when the artery walls narrow, or constrict. Blood pressure is highest when the heart beats to push blood out into the arteries. When the heart relaxes to fill with blood again, the pressure is at its lowest point. Hypertension is a major health problem, mainly because it has no symptoms. It is serious because people with the condition have a higher risk for heart disease and other medical problems than people with normal blood pressure. Serious complications can be avoided by getting regular blood pressure checks and treating hypertension as soon as it is diagnosed. It can lead to the medical conditions like arteriosclerosis, heart failure, stroke, enlarged heart or kidney damage.

Heart failure is usually caused by the effects of some type of heart disease. Heart failure is a condition in which the heart has lost the ability to pump enough blood to the body´s tissues. With too little blood being delivered, the organs and other tissues do not receive enough oxygen and nutrients to function properly. People with risk factors for coronary disease should work closely with their physician to reduce likelihood of heart attack and heart failure. When the heart cannot pump enough blood, it tries to make up for this by becoming larger. By becoming enlarged (hypertrophic) the ventricle can contract more strongly and pump more blood. When this happens, the heart chamber becomes larger and the muscle in the heart wall becomes thicker.

The heart also compensates by pumping more often to improve blood output and circulation. The kidneys try to compensate for a failing heart by retaining more salt and water to increase the volume of blood. This extra fluid also can cause edema. In due course, as the condition worsens over time these measures are not enough to keep the heart pumping enough blood needed by the body. Kidneys often weaken under these circumstances, further aggravating the situation and making therapy more difficult. Most patients with mild or moderate heart failure can be successfully treated with dietary and exercise programs along with the right medications. Patients with severe heart failure may eventually have to consider heart transplantation.
